L-PLLA (poly-L-lactic acid) is a biodegradable, biocompatible synthetic polymer widely used in medicine, including aesthetic and regenerative medicine, as a proven biostimulant.

In clinical practice, L-PLLA is used in certified medical products such as Sculptra® and Olidia®, designed to gradually improve skin quality by inducing neocollagenesis.

Mechanism of action

After injection into tissues:

  • L-PLLA particles initiate a controlled biostimulatory response,
  • fibroblasts are activated and type I collagen synthesis increases,
  • the product biodegrades gradually into lactic acid, then metabolized to CO₂ and H₂O in the Krebs cycle.

The clinical effect is indirect and long-term, resulting from extracellular matrix remodeling rather than a direct filling effect.

Clinical indications

L-PLLA treatments (e.g. Sculptra®, Olidia®) may be used in patients:

  • with loss of skin firmness and density,
  • with signs of biological aging,
  • after significant weight loss,
  • for prevention of skin aging,
  • seeking gradual, natural regenerative results without changing facial features.

Most common areas:

  • face,
  • neck,
  • decolletage,
  • arms,
  • selected body areas requiring improved skin quality.
Therapy outcomes
  • gradual improvement in skin tension and elasticity,
  • increased skin density,
  • improved structure and quality,

Results build over time (usually weeks to months) and can last for many months, depending on individual factors and the treatment protocol.
